Office Policies
 
Appointments
  • Patients are seen by appointment only.

  • New Patients are asked to arrive early to fill out paperwork.

  • Same day appointments are available for patients with acute illness on a first come, first serve basis as the schedule allows. The time allotted for these appointments does not allow for additional concerns to be addressed at that time.

  • Established patients who arrive 15 minutes late for their appointment will be seen after those who have arrived on time, if the schedule permits. Otherwise they will be asked to reschedule.

  • This applies to new patients who arrive after their scheduled appointment time.

  • Patients with chronic conditions (such as hypertension, high cholesterol, diabetes, kidney failure, or anyone on long term medications) require regularly scheduled surveillance of these conditions.  Therefore, routine prescription refills will require an appointment with a provider every 3-12 months depending on the condition.

  • At times, the provider may be required to attend to an emergency
    during the time of your scheduled visit. You will be informed of any
    delays, and we appreciate your understanding should this
    situation arise.

  • If you must cancel or change your scheduled appointment, please
    call us 24 hours prior so that we may accommodate other patients.
Prescriptions
  • Please bring all prescription bottles or a current list of all medications with you to each visit.

  • To obtain refills of your routine prescription medications, call your pharmacy 72 hours prior to running out and have them contact our office for refill authorizations.

  • Please allow 3 business days for refill requests to be completed.

  • Prescriptions will not be refilled on Friday afternoons. We suggest you check your medications prior to any vacations, holidays, and weekends to assure an ample supply during these times.

  • Antibiotics will not be prescribed without being seen by a provider.
Hospital Admissions
  • The Providers at Cornerstone Family Practice use the Zenith Group
    of Hospitalists to admit our patients should the need arise.
Confidentiality
  • Your personal and medical information is strictly confidential. No information regarding your condition or medical treatment will be given to anyone without your written or verbal permission. This includes spouses, friends, relatives, or employers, except when required by law, in accordance with HIPPA Guidelines.
Narcotics
  • Providers at Cornerstone Family Practice  DO NOT routinely prescribe refills for pain medicine, or other controlled substances.  If you are seeking these services, this is not the practice for you!

  • We do require a full set of medical records in hand at the time of the initial office visit for any narcotic prescriptions.

  • Any patient who receives a short term appropriate controlled substance is required to sign a narcotics agreement with our office and a DEA report will reviewed monthly.

  • We refer patients who require long term narcotic medication therapy to Pain Management Specialists.
Billing
  • All charges incurred are ultimately the responsibility of the patient.

  • We will bill your contracted insurance company as a courtesy to you.

  • All contracted insurance co-payments are due at the time of the visit.

  • If we are unable to verify your insurance coverage, payment in full is due at the time of service.

  • All previous balances are due at the time of service.

  • Charges for all Laboratory Studies or blood work obtained in our office are the PATIENT”S RESPONSIBILITY!  We have no control over what your insurance does and does not cover!  Providers order tests based on what we feel is medically appropriate.  Insurance companies don’t always agree.

  • We will not, under any circumstances, change diagnosis codes, or alter the medical record for sake of insurance company reimbursement.  This is called Insurance Fraud!

  • Immunizations such as the HPV and Shingles vaccine are collected in full at the time of the administration of the first shot.  If your Insurance reimburses, subsequent injections will be billed to your insurance company as a courtesy.
